POSITION SUMMARY
The Reentry Specialist is a non-exempt position assigned to the York County Department of Probation Services, which staffs the York County Reentry Coalition. The Reentry Specialist is based at the Reentry Opportunity Center (ROC) on the campus of York County Prison. These duties include referral management, coordination of providers, data collection and analysis, database management, creation of statistical reports, planning and outreach document creation, communications, and administrative functions such as recording of meeting minutes, scheduling, and correspondence. This position provides a number of essential liaison duties to the justice system in interacting with community service providers and health and human services agencies. This specialized position works at the direction of the Reentry Coalition Executive Director and is responsible for the day-to-day operation of the York County Reentry Opportunity Center (ROC), assisting to create collaborative partnerships, data collection, and statistical analysis of data from a wide variety of sources.
ESSENTIAL DUTIES, AND RESPONSIBILITIES
The duties of the Reentry Specialist include:
York County Reentry Opportunity Center (ROC)
Coordination of the York County ROC day-to-day operations, including reception coverage, creation and distribution of service provider schedules/calendars, and client signs ups. This includes coordination with the Work Release Unit of York County Prison.
Participation on the Reentry Multidisciplinary Team (RMDT), including data entry and accurate maintenance of statistical data.
Triage referrals and connect clients to appropriate care with program partners, to include referral tracking and follow up. To do this, the Reentry specialist will partner with staff at the York County Prison, the York County Office of Mental Health & Intellectual and Developmental Disabilities, WellSpan Philhaven’s START clinic, the York County Coalition on Homelessness, and a variety of health, human services, and reentry organizations. Maintaining a knowledge of county wide service providers is essential to perform this function.
Assertively oversee and troubleshoot referral processes, including following up with referral agencies, monitoring client progress, and providing necessary health and service information in a timely manner to relevant justice agencies.
Assist in development of referral partnerships and reentry programming based on needs of reentrants.
Assist in developing community partnerships to ensure access to resources by reentrants and their families.
Assist in tracking of short-term, intermediate, and long-term impacts of services by monitoring recidivism rates over time, overdose and overdose fatalities of clients, quality-of-life improvements, and other designated measures.
Coordinate meetings and community events, including set-up for events.
Maintain compliance with all state and federal privacy regulations.
Maintain accurate daily records, reports, and files.
Provide administrative support to staff and service providers at the Reentry Opportunity Center to ensure compliant and efficient operations including occasional evening hours.
York County Reentry Coalition
The Reentry Specialist assists the YCRC Executive Director with:
Tracking of progress toward the short-term, intermediate, and long-term goals of the YCRC Strategic Plan.
Maintenance of the YCRC membership, mailing lists and referral database.
Coordinating and convening YCRC committee meetings as needed.
Providing set-up, tear down, and general support for YCRC programs, trainings, and group meetings, including virtual meeting platform operation for Coalition meetings.
York County Probation Services Reentry Specialist
Creation and distribution of communication materials (e-newsletters, flyers, posters, social media posts, website language etc.) to raise community awareness and engage stakeholders.
General clerical duties including but not limited to answering phones, photocopying, scanning, filing, mail management, supply maintenance, purchasing and office organization.
General Requirements & Skills
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Ability to exercise sound judgement, work independently, and handle multiple tasks through effective time management in compliance with the policies and procedures of the Court of Common Pleas and the York County Department of Probation Services.
Ability to excel in collaborative work environments.
Ability to work well under pressure, troubleshoot and proactively manage stressors through self-care, including maintaining a calm and sensitive demeanor in stressful situations.
Intermediate proficiency in computer programs/data processing to include the Microsoft Office suite (Word, Outlook, Excel, PowerPoint required. Publisher, Visio, and/or Tableau a plus).
Literacy in data collection and analysis, including mathematical and statistical concepts like rates, ratios, averages, median, and mean. Familiarity with US Census data sources a plus.
Familiarity with government and community-based health and human service providers in York County and surrounding areas.
Understanding of case management functions and ability to coordinate services across multiple agencies.
Familiarity with essentials of trauma-informed care and/or mental health first aid preferred.
Ability to communicate effectively verbally, in writing, and in graphical illustrations.
Strong group and meeting facilitation skills.
Ability to maintain positive and professional relationships with diverse groups, community representatives, members of the public, justice involved persons, peers and partner agency staff and leadership.
Ability to maintain confidentiality in all aspects of the job.
Proficiency in operating common office equipment, including computer, printer, scanner, calculator, copier, and facsimile.
Proficiency in Microsoft Office operations Required. Proficiency in Google workspace desired.
Ability to work around individuals currently incarcerated required.
Ability to obtain York County Prison security clearance required.
QUALIFICATIONS
Associate degree in Criminal Justice, Public Administration, Psychology/Social Work, or a related field, or five years of an equivalent combination of education, training and relevant work experience required.
Criminal justice and/or human services work experience preferred.
York County Probation Services Reentry Specialist
A criminal background check will be conducted. Individuals with lived experience with the criminal justice system, substance use & recovery, and/or mental health & recovery are encouraged to apply.
The successful applicant must receive applicable child abuse clearances prior to employment.
Bilingual (English/Spanish) preferred.
